It happens to me too, it's rare and random (Self-hosted).

Using a custom theme (not the default one) along with 2/3 plugins/apps, difficult to determine where the anomaly comes from.

I was thinking a quick double click and a temporary server slowdown.

CiC here. Pretty much the same here, running a slightly altered version of the default. However, switching to the default theme, it still displays as having been reacted to twice by the same user.

Yes indeed, when I checked in the database, there were indeed 2 reactions for the same post and for the same user.

Unfortunately, this isn't something I am able to reproduce on the core software. Is this something that you are able to reproduce with all third-party applications/plugins disabled and on an unmodified theme? If so, could you please let us know the steps you took?

A user can react to something, remove it, and use the same reaction again or change it to something else. 

We have seen reports of this in the past on self-hosted environments, but unfortunately have never been able to actively replicate the issue

The user there actually reacted to 2 different items there (2 posts in the same topic)
