It checks members post count. Meaning if the post count is converted over, it will be conted/recognised. They will not be forced to go through the approval process for their initial post on Invision.

You have installed Ipb 4.5.x in the new server and uploaded the database copy of your 3.4x forum at the same database of 4.5.x forum? Did I get it right? If so, you must run the upgrader to bring the database up to date with the new version, 4.5x. But note that like this you will loose all attachments/avatars etc.
You say that you are moving hosts too. The best way to go about this imho, is to first move your 3.4. forum to the new host and do a proper upgrade there afterwards.

You should not run the installer, but the upgrader. Did you overwrite your 3.4. files with those from the latest version of 4.5 package? Can you please outline step by step everything you did?
If you do not want to loose content, meaning restoring the backup, you can have that member re-register, or create an account manually for him/her, then you can re-assign their posts/topics with a few sql queries.
You are very welcome :) I just submited a new version with the fix btw. I have also implemented your suggestion about adding in the settings the text that the topic authors will be excluded automatically from being forced to read their own topics.
Are you the topic starter of the topic that you have set as must read? Because if you are, you are excluded automatically as there is no point to force members to read their own topics.
Thank you for reporting it. I will make that option as not required and release a new version soon. In meanwhile, you can enter a topic name for guests, it will not run as long as Force Topic Reading for Guests option is disabled.
Hi. This app does not suggest to members to read a topic, but instead it sets a topic as must read and members will get an error when trying to enter the forums and the other topics informing them of that fact. i.e. it will force your members to read the topic set by the amdin as must read.
The error message you see is the standard one added by the app. It can be customized. You can use html should you wish to do so. Please see the attached screenshot. (The field that says: Error Message).
For your first request, I have not seen such a plugin/app myself. You can request one to be made for you at the request forum.

For your second request, it does not exist in the core, but I have seen a couple of apps that doe that. One by @Adriano Faria and one by @DawPi.
Yes, that is possible. You can also have a log in the Admin Panel to see who verified whom and what time did they do so. You can have a reason field as well so they can say how do they know the member that they verified. I have done something similiar not so long ago.
This seems to be permission related. Do you have your own Vps/Dedicated Box or are you on a shared hosting plan? If the latter, you should contact your host about this.
