Using a custom theme (not the default one) along with 2/3 plugins/apps, difficult to determine where the anomaly comes from. I was thinking a quick double click and a temporary server slowdown. CiC here. Pretty much the same here, running a slightly altered version of the default. However, switching to the default theme, it still displays as having been reacted to twice by the same user. Is this something that you are able to reproduce with all third-party applications/plugins disabled and on an unmodified theme? If so, could you please let us know the steps you took?
