Default profile was deleted after update

Hello guys,

 

We had this problem that the default profile photo was deleted or somehow removed or gone.
Is there a possiblity that it somehow gone or removed after update?
Im pretty sure it was not deleted as I was the only person responsible updating the IPB and for sure I did not removed it.
Our default profile photo is a custom image, so just wondering what might cause it?

Just normal upload in the themes > manage resources

However the staging was not affected, as we had this SOP that we update staging first then if no problem, we update IPB production.

Just now our team notice it and I took care of it, so it was fixed by uploading again, however I did not know the root cause, so I thought to post it here and maybe there are some experienced member who were able to pinpoint the root cause and prevent it from happenning again.

I take it that the image was somehow deleted/removed. Can you check the server/access logs and see if there is anything logged in there pertaining that custom image?
