The notification is sent only when it is approved and the author sr it as ready via ALLOWS PLAYS. Check the secondary groups for the first member ir moderator permissions. He got permission to bypass moderation somewhere.

No. Allows plays is just an internal flag that will check if the quiz has questions and answers. Make a test yourself. Create a member in sane group or log in on someone else’s account and try it. Let me know. 👍

It's not a setting. It's a group permission. Edit the group and enabled it for Members.
