Error code: EX256 Invision Support won't help because we're not on the latest release (not ready to move our server to PHP 5.6). Any ideas how to fix this? I suggest you to disable all 3rd party apps and plugins, one by one, and go testing, until you find the culprit. You can also check the logs, as mentioned above.
