I think I got the point, guys... not working with 4.1.19. It was just released 21 hours ago. A little patience please.
From what @Meddysong says, this is being caused by a bug in 4.1.19. You'll either need to upload the patch file given here to fix it, or wait for 4.1.19.1. Thanks Meddysong.
 

There's a known fault in 4.1.19 which is causing problems with some plugins, so there's nothing Ryan can do. You either need to apply the patch or wait for 4.1.19.1.

I don't know that that is related to your issue of the prefix disappearing with the app enabled. That's more likely to do with app settings, or a theme conflict or something else. I don't recall without taking a close look.
That being said, this app doesn't work that way. You can't enter any tag and choose one as a prefix with the app enabled--all prefixes must be predefined and enabled for a forum, and then you can choose one from a dropdown. The app description and screenshots make this pretty clear (I hope), and it is what most people who buy this app are looking for.

There is a tag cloud widget you can put on pages. There isn't currently a dedicated page listing out all tags.

In the ACP > Community > Advanced Tags > Settings : disable "Add topic prefix to the page title"

Feature suggestion: 
Export list of tags in XML / TXT format -- I have 71 pages of tags and growing, and eventually I need to be able to clean up some of these tags.  Being able to export to an XML and then cutting / filtering the tags in Microsoft Excel so I can provide a master plan for all of the tags would be a huge benefit.

I take a look at that and the difference is that when  Advanced Tags & Prefixes is enabled the <span> of prefix contains the template Core > Global > Prefix

I'm not entirely sure what you're referring to. The app listing description? There's a tag manager tool that allows you to view all tags, from which you can edit or delete all instances of a given tag/prefix at once.

I spent a while exploring it. Found the source of the error, but not a solution that doesn't break AT&P in the process. There's some really bizarre behavior going on with the IPS core here. I reached out to someone about it.

I don't do release tests, but assume yes unless anyone indicates otherwise.

Understood, thanks. I'll look into it for the next update, but otherwise, unless you see something break, you can disregard the "DB errors" the support tool reports. The support tool is known (notorious) for misinterpreting the database schema, writing it one way and then reading it as something different, when it's actually the same. Evidently they changed something about it in 4.1.14, and this was a side effect.

For anyone else, you can pretty much just change the pages/{@page} FURL template to pages/p-{@page} (or any other static text in the second part) to work around the issue. That will mean all of your page URLs change, though; for the moment, that's unavoidable.

I found it, its under the system app and search permissions for guests. I realized this after seeing that the tags pages were search results. google sees these are 404s if you don't enable this. Its good for internal linking. I don't really like guests searching but in this case, I need the SEO. I decided to use flood control for guests and set it for 25 sec. I changed the language settings for the error page they get when they have to wait to:
Please wait {# [1:second][?:seconds]} before attempting another search. Avoid waiting the extra time by logging in or registering as a member of our community.
I figure maybe that'll prompt registrations.
