Outgoing database links come from a theme template called “basicRelationship”. Incoming links are defined in the display template assign to the database. So, that would be the most straightforward way to change the comma-separated lists to something else. 
Of course you can also do it all manually and load the records that are saved in the database relationship field—provided you are building a custom template that is not meant for multiple databases. 

Yes, sounds like a perfect case for a Pages database. The custom fields you can set up in the ACP also come with a few layout options. Check the Display Option tab for the fields. For images, you probably want to create separate upload fields, not just rely on an editor field with attachments.

We are currently using more or less the default set up for achievements, but have excluded some larger groups (only paying groups will earn achievements). The 'rare' setting seems to be set based on the total membership base, including those groups that are excluded. Is this correct? Shouldn't 'rare badges' be calculated based on the included groups?
 

Correct, it's based on the total members count.

I agree, that's a fair point, which I have forwarded to our internal suggestion list.

On 4.2 version there's a button to allow people play the quiz. The quiz will only be "playable" after the submitter click on it. Topic is still on quiz creation but I can change it too.

Just bought this and installed it locally on a test-forum. Created a couple of categories and created an XML for import. When I go to import I cannot choose category. Not from Site either when trying to create new quiz there. The categories are enabled, but seems to not be picking them up? Probably me that has missed something
EDIT: Sorry, simply had to check the permissions on the categories. All good now
