Sounds like an issue with a theme hook.

How can I resolve this theme hook issue?

You would need to contact the author of that theme for assistance

Or indeed the author of the plugin, if It's a plugin that is using it

We are using IPS default theme with just a few edits and currently NO plugins.

It would indeed then be your theme you would be looking at
