Since upgrading to version 4 I've been getting the following emails a few times every day. I have no idea what the problem is and what I'm meant to do. Can anyone explain what I'm meant to do and how can I stop the emails.

 

 

I am getting a bunch of them as well.  Haven't begin to delve into it though.

I got a load of them last night, during a 1000 post korean spam wave. I suspect it's accounts that got caught by the IPS spam filter, but then they've still tried to validate the user - or they've tried to validate the user more than once. I've been too busy cleaning up spam to investigate further.
