Well,i can only talk for IPB 3.4.6, but when i was changing the default one, it was affecting immediatly all other (without a custom one).

I think in IPB 4 is the same...

Ok found it but it didn't change the existing ones. Does it only apply to new users since the image change?

​You likely have gravatars enabled, which means it must actually retrieve each avatar with a custom string via the gravatar-service, even if they just use the default avatar. 

Since each member have their own avatar-"identifier" in the gravatar case, it means that each of them will be cached individually in your browser and/or on the gravatar service. Which in this case would explain why you see the old one for previous members still, because they are cached, either on their service or in your browser. While you obviously don't have the new members avatars cached in your browser.

On IPS4 you change the default avatar via theme resources for your specific theme. The link is in the dropdown on the side of each theme in the themes page. It's also not in the /public/ folder which is no longer used.

This.  You go to Customization in the ACP, click the dropdown for your theme, then you go to Manage Images and replace the image from here.

In 4.0 you should never try to just replace random files on disk.  With the way files are tracked and mapped in 4.0, this will rarely work correctly (much like you shouldn't edit template files stored under the /cache folder in 3.4).

​I have followed this procedure. On theme's page in AdminCP the new image is ok, but in the forums the new image is not loaded. It is not found by the system (404).
