BB code support is turned off by default in 4.5. Just post the image URL itself. Wrapping an image URL with bb code isn’t needed anymore for years.

It hasn't been fully released yet; current release is 'soft' meaning it is available for some customers, not all.
If you really want to upgrade, you can do it manually by downloading the package from the client area, uploading it to your server and then initiate the upgrade manually.
