Has anyone every encounter the issue, duplicate username and email address on registration?

Check out why avatars are different?
If the images are different, this is your humorous user. Probably the letter B, a, e, is with Cyrillic , and it is also possible that this user has a similar email address using the Cyrillic symbol.

The avatars are different because I have an avatar generator that gives the user a random avatar on signup. both duplicate was created at the exact same time, similar to bug that would create a duplicate forum post.

No, that should not be possible and may lead to issues down the road. If you can't figure out how it happened (do you have any third party login handlers, custom plugins that affect registration or member accounts, etc.) you should submit a ticket.
