Hello Phil, No to all. As stated, it allows the user to add books (records) using data from Google Books. There will be copyright issues allowing to upload/download books. Each country may have its own regulations, etc. Of course someone can upload their own books and offers them to download but I’m pretty sure this will be an exception, so no plans for this on a near future. Need to give some thought on this as I don’t have much information right now.

Yes, you choose a type when creating the quiz. A lot of things happens in the background, like different ways to calculate time and score, so there’s absolutely no way for me to change that.

Not exacly. You don't see, even as admin/moderator, the content posted by guests via POST BEFORE REGISTERING. They only get visible after the account is created. I mean, you can view with the following plugin: https://invisioncommunity.com/files/file/9296-post-before-registering-content-viewer/

You probablly has POST BEFORE REGISTERING enabled then guests has MEMBERS permission and will be prompted to create an account. BUT if the posts is authored by GUESTS yet, then that's not the case as the posts would be visible only after the account is created.
