Custom forms

Is there an easy way to create custom forms? Looking to do something like the attached

What should happen after the form submission?
You could use our Pages App and store the data as records, or use one of the few 3rd party apps from our marketplace

They need to be sent to 2 email addresses. Don't have a need to store them as such.

I was looking at both those forms but wasn't 100% sure if they would work. 

Also I want something that I can just drop into the source code of a customised page.

You could use the menu creator to point to a specialized webpage and embed a form tool inside of it, you could use member groups to allow/disallow access to the page.

I'll move this over to community support for you as there may be others who also have more ideas. Its not something we have within the software outside of the pages application solution mentioned. But that doesnt sound like what you are going to need here
