Im trying to figure out how make applications like this for my website.

 

The picture below shows what the application is and the template for it.

 

 

The picture below shows the forums they automatically submit too once they finish the application. 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

The picture below shows what the forums posts looks like. As you can see it posts their input under the questions.

 

So what im trying to figure out is how to do something like this for my website. Do I need a plugin or what?

Here goes one: 

You create the position, choose the fields you want the applicants to fill and then all you need to do is manage the applications by approving or rejecting it.

The application itself is restricted to moderators with proper permission.
