Is it possible to create a database and associated page to capture user info and pre populate some of the fields based on the user logged in?

Technically speaking, yes, it is possible. You'd need to use a custom form template on the database or at worst hook into the database-specific records class. I haven't actually gone in and done something like this, so I can't speak for the difficulty of it however, and such is not a built in option for a field default value.
