Honestly, I don't know of any. Free hosts tend to not offer MySQL databases.

I know of one, but considering that it's not permitted to give out links to other companies for hosting purposes, I'll keep it to myself.

I don't know of any but you can purchase good hosting options that cost less than $200~300 a year. (shared server)

I only pay 6 bucks a month for plenty of resources. Never had any problems.
