Enabling a subject on the Contact Us form?

We redirect our Contact Us form (for guests) to created Support Requests. Right now they all come through with the title of "Support Request created via Contact Us Form".

We would like to offer an optional subject line on the contact us form. Seems a simple request, but I cannot find the setting?

Did you already try to create a custom field on Support > Support Settings > Custom Fields > Create New? Don't forgot to set the department area, where will display.
