4.0.6 broken record images?

After upgrading to 4.0.6.1, any record images I upload won't display because the code is:

<img src="monthly_2015_05/image.jpg.2c18f1540fa4512ea167fe5a4d718722.jpg">

Instead of:

<img src="http://mysite.com/uploads/custom-path/monthly_2015_05/image.jpg.2c18f1540fa4512ea167fe5a4d718722.jpg">

All the existing images are fine, it's only new ones. Anybody else had this issue?

​I've the same issue after upgrading from 3.4.6 to 4.0.6, do I have to modify the current default template?
