Unable to create New Topic/ Reply in the Forum

I am  facing some problem in the forum.
1. No one is able to start a new thread in forum. Please check the attached Screenshot. (Create New Topic.jpg)
2. Approve Button is getting pop-up. Please let me know how to remove it?
3. I am unable to reply to the post as the box. Screenshot - (Approve Button & Reply Error)
4. I would also request Technical team to check whether the installed version on forum is latest or not. If it is not latest please let me know how to install latest version.

For some reason, you’re missing an important JavaScript file. This is why the text field doesn’t expand, and why you’re unable to post replies. Try running the support tool from the ACP (System > Support > Get Support), and clear your caches:
