That's what I thought until today, however it appears to be shown or hidden dependant on two things: (I just found the control that changes this after I made my post...) If the user's attachment upload ability is disabled or they have reached the maximum storage limit then the control changes: This means the ability to 'insert existing attachment', which includes from the gallery, is hidden based on the above settings. (I specifically disable attachments but make use of the gallery, so this is quite bad...) Every day is a school day.
