Editor Bar Icons Wrong

Various users keep telling me (and I've experienced it once) that the editor bar icons get "off"... meaning if you mouse over, the description of what the button does, is not what it actually does.

Ask the users to fully clear their browser cache or try in another browser and see if the issue persists. Usually this is due to an out of date cache.
