Media Promotion - Facebook Permissions

To use the new Media Promotion feature, we need to add 2 new permissions to our Facebook app. Publish_pages and manage_pages.

How did you set up those permissions for facebook review process?

They even ask for a screencast. Can someone that successful was approved, show the screencast used?
