Skin URL Remapping

Requested features:

1) Enable/Disable, not just Add/Edit/Delete. Could come in handy in skin tests (e.g. disabling an IPC-page-specific skin temporarily to test another skin on it) or temporary/seasonal events.

2) Display the 'Type' and 'URL' values in columns on the main URL Remapping page, so we can more easily determine those values without having to go to each specific Mapped URL page.
