Huge ass in the header of every page… this thing makes our form look like crap it suddenly appeared out of nowhere and was not answered by us can you please help remove the ad at the very top of the site. It is in the top of every page and it looks like someone hacked my head put it there. Please help remove it

Wild guess: You use Google Adsense and they started an experiment on your site. In this case, you would turn it off in the Adsense settings.

Adsense: I am guessing you turned auto ads on. Also, page level turned on.... my best guess....

Gruesome! 

I saw something similar when experimenting with Auto Ads.  I was able to work around this by placing a manual ad unit just below the header.  Google won't place an auto ad in the spot it normally tries to at that point.  However again, if Google cannot (or will not) serve an ad...  it simply displays a blank spot.  So think carefully about your ad placement and use manual ads to make sure auto ads don't ruin your page (or don't use it at all!)
