Layout Errors with newest Update

We just recently updated to the newest version and in doing so our layout has a few errors that we are noticing.

Firstly, the image for the user who posted content is displayed twice as shown here: 

I was able to find that there are two identical lines of code being written for each post that href back to the users profile (as they should) but I am unable to find where to edit to be able to stop the code from displaying the image twice.

 

Secondly, the user who posts the content is being displayed as the reply-er, and the reply-er is being shown as the poster. These just need to be switched, but again, I am unable to find where to fix these issues.

 

Any help that can be provided would be great.

Do these issues exist in the default theme of IPS?
If yes - create ticket support in your client area.
If not - your custom theme has outdated templates and should be edited.
