Custom Tool Bar Button HTML

I would like to add a custom tool bar button that pops out like the Emoticon button which contains the code in the embed box at the bottom of this link http://www.pokerhandreplays.com/converter-embed.php

I haven't managed to accomplish this with the "Add Button" feature under Toolbars in the admin cp.

Any help would be highly appreciated,
