Moving everything to new hosts

I have had constant problems with our current hosts and have now decided that i am going to move everything to a new host. Now i have never done this before and want to know whats the process for me to move everything from my current hosts to a new hosts, i want to keep all my posts, members, mods, ect.

I understand HTML, CSS but have never worked with databases apart from creating new ones. I need the guide to be as simple as possible please.
