This error indicates that the file is missing.

Is this an upgrade test? Did you copy ALL your files from the directory where your downloads files are saved, if you're not using anything else like S3?

It happens with every new file submission. when I attempt to upload a new version (update/ upgrade to a new file version).

We are unable to replicate in here. As asked by my colleague, is this an upgrade test of an existing site?
