Custom Fields in Reviews and Comments

Is there anything out there that allows admins to add custom fields to a 'Write a Review' and/or 'Reply to this topic...' forms?

I found a few plugins that add custom fields to topics, clubs, calendar, etc forms but nothing for when leaving a comment or review.

If nothing available I will pay to get this done.
