What other custom fields might you develop?

Images in Gallery already support overlaid "notes" which you could use to tag users, although this won't act as a 'mention' and notify the user or show that any specific users have been tagged. Tagging users is a bit specific and would probably be handled in a better manner than a basic custom field, so I'm just trying to see what other use cases you might have.

Maybe location?

And what about this function: make the photo anonym for 24h after loading. For more independence rating of photo)
Only administrators, moderators and owner see the author (or other groups - via settings of gallery).
And there is ability to set this function per caterory.

Examples of custom fields that i can foresee on my own site:

Studio or Publisher

Magazine

Season (like Spring / Summer or Fall / Winter collection)

Date of first printing

Checkbox if the photospread is a later revision or update

Etc.

Yes, Gallery should be more explored by IPS. It is an app with great potential that is forgotten due the lack of links to other apps and lack of custom fields, for exemple.

Is it possible to extract the location exif metadata of a photo and show a google place with Gallery current features?

Not out of the box, no - there is no "consolidation" of locations into a single map. Each image shows an individual map (if the image has GPS info embedded and the option to show the map has been enabled).

i think it would be really useful to have an option to report an image as spoiler/explicit content and hide content with a blur effect or other in thumbnails until you hover over (Steam style) or open content.
