Attach notes to a topic

I think it'd be useful if moderators and admins can leave notes in a topic. These would be visible to them only. Perhaps add something view them to the dropdown in Moderation options or have the notes show at the beginning or end of a topic (using Ajax). Hope this makes it into 3.1. :D

Theres already a mod for this in ipb 2.6

There should be another mod released for 3.0 as well.
