How are you adding the categories ? I don't seem to have that issue are you using the latest version 5.0.5 beta 2 ? are you adding sub categories to the main category then add a child category to the main category image below this is how I have mine setup this is how it looks on main site Not sure what the problem is on your end but everything is working as it should on my site. Hopefully you have it solved I can delete mine.

I did that enabled rewrite urls and downloaded the file renamed to .htaccess uploaded to the root and it seems to work you can do that or wait for the next beta release.
