If I modify, import or create a new "Index" template.

Assign that "Index" template to the database.

I get this bowser error when I try to view the Pages app articles home page when the site is in DEV mode.

BadMethodCallException

NO_TEMPLATE_FILE - /Users/Beats23/Sites/My-Site.com/applications/cms/dev/html/database/custom3/index.phtml

Yes, it's always worked that way. If you're working in development mode, you need to create the templates in your dev directory, same as you would for other templates.
