How to creae form to assign points?

Hello, 

I will probably need a plug-in for this but I couldn't find anything related.

What I would like to do, is to create a form where I can choose if a certain member gets a certain custom point and not to go to each profile manualy

For example, I have a custom field in a profile that is called "Practice Points", the answer is numbered (1,2,3,4....)

I would like to have a custom form where I have a list of members, and then I can just press if a certain member gets a point.

 

Is there something out there that could help me with this?
