Creating Forms

Hello.

i hope I make myself clear on what I'm looking for. I'm wondering if there is a way to create a standard input form for clients to post to? For a select "client" group (not all our groups), I would like these clients, when they reply in their personal "journal", to have a standard input form come up automatically for them.

We have communication issues with clients not providing us the details we need for our support program, so this form would provide them with the reminders for the information we require on a daily basis.

If this cannot be done as is, I would like to find out if it's a feature that could be contracted for me?
