Are they showing if you have the "User suite HTML wrapper" switched on?

No, if I have the "User suite HTML wrapper" switched on, there is no further options to select.

Sorry, I feel there may be confusion there. If you choose no HTML wrapper, you are saying you are writing the page yourself, so you would only have custom template, as thats what the setting is for.

If you choose to use HTML wrapper, then you are designing your page on the front end. On the assumption you are asking because you were using version 4, it doesnt work like that in version 5. You can drag things into columns on the front end of the site.

This is working as intended. The single/two/three column layouts are deprecated and are only present if you have a page that is already using them.

With our new Page Editor, these layouts are unnecessary and only cause confusion within the system.
