Currently this is how all my tags and featured content pages looks like on my test forum (version 5.0.2),

Only when I "feature" a content I see an option to add "feature image" for that topic. Is there any other way to add an image that Im missing ?

Reason Im asking is my online forum (version 4.7.20) has over 500 featured content and looks like if I update only way to add images to existing topics is "unfeature" > "feature" and add image which looks very painful 😑

You are mixing 2 things there. Tagged content will use the first image in that tagged item

Above image is from a tag page and it still doesnt show any image ? All topics listed there have a posted image by the way.

And for featured content page, when I feature a topic I must upload a "feature image" manually right ? It doesnt use the first image in the topic ?

For the featured content one, yes that is correct. Do you have a link to that tag page?

I can not provide a link since I installed and updated my forum on localhost with xampp.

I even crated new topics with images in first post and they still doesnt show any images in any tag page.

It was an url copy/pasted.

I just created a new topic and uploaded an image as an attachment and it works.

Looks like image thing is not going to be for my forum because we already have over 35.000 topics with copy/pasted images.
