Adding mandatory tag drop-down fields for new topic

Hello,
Today in my forum, when a user opens a topic,
I use one required drop-down field to choose prefix (Radical Tags great app by @Makoto ) and one mandatory open tag for users to insert input.

I want to add several mandatory drop-down tag fields for new topic; how can I do it? I didn't find a suitable app.
