You could set your board to NOT allow members to change their usernames. If they wish to change it, they can contact you and you can then check the ACP to make sure it's not an old banned members name.

Just wondering, is that with the banned username already in the ban filter?

No, I do not manually put a user's display name into the ban filter after I ban them.

I would recommend submitting a ticket. This should definitely not be happening.
