Could it be because during the upgrade, the software asked me to manually run the MySQL commands? they already exist because I added them manually.

Those errors are coming from the upgrade process. Yes, they will likely have been because they have tried to be run twice. As you are now upgraded though, you dont need to worry about those

Concerning the theme editor bug, it occurs when you open the theme editor with this setting is turned off.

It was off by default on my upgraded site. You can't use your account until you turn it on again.

You need to check your server error logs on that one. If you are getting 403, then your server is actively denying access somewhere
