I see what you mean on the images. In all honesty though, its just a default example set, and could be changed to anything you really wanted to add.
The descriptions you are looking at there like "Started a discussion" and the likes, are achievement rules which state why they have been given, so you would look at the rules section, rather than the badges section for those

Ranks are customisable. Im not sure what it is you are seeing as an issue there in item 1. Indeed our are different to your own, and thats fine. You can of course change them in members->Achievements->Badges if you wish to use a different image or create a custom image with the build in badge creator. This is also where you would change the names of them for item 2
