We can certainly take a look at the second item there, but could you please explain what you mean on the first? I ask as you are showing where you would add a custom icon, and show that you have added one.

I have added the icon for the announcement forum, but it's not showing there. Am I doing something wrong?

It could be related to the same issue.

Some of the thumbnail images are also missing from the right sidebar

Are you using any caching on your server at all? If so, please could you disable that and check again?

I've checked and found that we're not using any caching.

What you are seeing there is the card image, which is not filled out.

The Icon would be shown when the system is in Table view.

Correct, table and feed view show the icon.

Is this issue fixed in 5.0.4 or just we are having this issue?

Sorry, is there any further question here? As you don't have a card image set, nothing would appear where you're using the Card-based views.
