My club leaders can no longer 'Add members'. They can invite, but not add. I've looked for relevant settings, permissions, etc. to no avail.

Inviting members would be expected in types which have membership options. There is no straight up add button unless you mean adding something else? It could be you were using a third party solution here prior which is no longer working.
