Are there other settings that control the Popular Now threads?  I have mine set up for 5 posts in 60 minutes but I have threads with 10 posts in 30 minutes and no popular now designation!  What could I be missing?

Yes.

I think I got it working...the widget was missing from where I thought it was.  Is the widget the only place the popular now topics appear?  No icon designation or highlight like a popular post gets?

Correct. Only the block (widget) would display this information.
