you have upgraded from 3.4.8 to 4.0.12.1 ?

yes, on local machine. 

okya i get the same issue after copying locally file of your 3.4.8 did your board was running correctly ?

if yes you have to take care of your upload path and upload url settings on 3.4.8 before the upgrade, because when you do a copy this settings are set to the original path of your original site so and after this 2 settings are ok you can proceed with upgrade.

Yes, before upgrade.

ok try to use before upgrade the 3.4.8 tools and in particular the upgradeFinish one it could help to fix various issues that are not really "live" at the moment.
