White Screen trying to install an application

Upgraded to 4.0.4, when I go to install an application, I am able to select the file when I select install I get a white screen.  Not sure where to look to see what the error is?

Try upgrading to 4.0.5.1?

I had 4.0.5.1 in test and there were too many issues.  4.0.4 was able to install applications in my test area, has to be some configuration or permission somewhere.
