Comes in 3 colors, a selection of pre-made icons and a blank to add your own. Also you can easily change the color, cause I´ve used a hue/saturation layer for the green and red icons. If you need help or anything, just ask.

Note: The font used is Myriad, I can´t put it into the zip, because its a paid font. You can easily change all fonts though...if you don´t have that font that is. ;)
