I've been looking at Forms for a long time, but since it did not previously integrate with new user registration I did not have much need for it. Now that it supports 4.3.1 and I can add it directly on the new user registration page, it was something I absolutely had to have!

Here is what I am looking for that would definitely make this a total game changer:

If the form integration fields on the user registration page could actually go into a database (not just forwarded or put into a log file), and have those records be part of the user account and have that be visible on the user's profile page that would be epic badass!

Presumably the user profile page part would need to be a separate companion product. But having it be configurable as to whether the user themselves could see it as part of the profile, whether everyone or just admins could see it, etc., would be totally amazing. I'd definitely pay (again) for that!

In other words, the ability to create (essentially) new user registration fields, have this be part of their profile, and having control as to who can/can't see it and which fields to turn on/off for display (everyone, user-only, admins only) is something Invision really needs and I don't believe there have been any 3rd-party solutions to this as of it.

Forms by Devfuse/Mike John is a very useful, powerful and flexible app to integrate all kind of forms in your suite. The app is totally integrated and works smoothly with your member management, staff management, site look-and-feel, and more. It will allow you to do many things you will never be able to do with external solutions, in place, all in a single app.  

One month after purchase we already have 6 forms on the way, and counting. We are becoming addicts. Our community is a voluntary association with activities IRL & online. We use forms for registration of paid membership, registration to IRL events, joint purchases, template for classified ads... Our staff has a relatively high turn-over ; permissions through groups ensure everyone has the means to work any time and also protect and preserve data, without painful external links and passwords sharing and changing.

Definite ++ :

  • All sorts of actions can be automatically triggered when a form is sent: private message, email, topic creation in a forum, with customizable sender and receivers. Created content include dynamically the form fields content, in whatever formatting you would like. All actions work perfectly as described.
  • A lot of form-by-form customizable possibilities (permissions, actions...)
  • All types of form fields you could dream of, including uploads and rich text editor, with perfectly working toggle on/toggle off possibilities between fields (including perfect management of the "mandatory but toggled off" fields.)
  • Definitely increases engagement of your users, who will feel being taken care of more personnally
  • Reliable and trustworthy developer with longstanding history in IPS Marketplace, open to new feature suggestions, regular updates, good support. By the way, a lot of comments in old reviews here have been addressed inbetween.
  • Robust, with very few (minor) bugs.

Things you should be aware of before purchasing :

  • Only users with admin privileges can create a form (from the ACP.) So, this part cannot be delegated to other members of your staff, except if you grant them admin permissions. However, all the rest (log management, follow-up of the requests...) can be!
  • CSV export of form logs is perfectible. For now, the columns of the exported .csv file are all the fields of all the forms, which can become quite large and unpractical. If your data management relies heavily on a csv file or processing an excel-type sheet, you might be disappointed. However, with all the possibles actions and the online logs, it is likely that you won't even need them.
  • For the moment, email automatic confirmation is sent by the creator's (admin) email address, with no possibility of customization. Beware if you don't want your email address to be disclosed.
  • Due to its power and flexibility, the app has a relatively steep learning curve in the very beginning if you are not an experienced user, or familiar with other apps from Mike John. Documentation is somewhat limited and phrasing is not totally self-explaining sometimes. You might need a number of trials and errors before achieving your exact goal (you will in the end, and then, you'll create forms at speed light.)

Overall: a must-have, totally worth the price!

This application is an absolute must for anyone running a community. From team applications to surveys, it's been an absolutely vital part of our website since we started out over two years ago!

Support is quick and effective, the author replies to pretty much any issue and has created manual fixes for us whenever we needed anything tweaked. The app is updated regularly and well, and we've never had compatibility issues either.

In our community, we have found it useful for various things, including:

  • Signing up to teams / staff positions
  • Signing up to guilds
  • Registering guilds on the website
  • Quickly creating forum posts from a template
  • A feedback form
  • Surveys
  • Incident Reports

And tons of other things! Almost every single thing eventually leads to us using a form :D

It's particularly recommended for people who need some of the basic functionality of databases but don't have the knowledge of how to create or manage them (which was us when we first got IPS), but even now that we have that skill, the forms are so convenient, and we use over 20 different ones daily, with many more on the side.

Plugin works great and does everything one could hope for in a simple easy to use package. I had a bit of trouble finding a list of the placeholders at first, however they're found in the 'Quick Tags' button under the Forms section. A tad annoying as I ended up having to take a screenshot for the first little while so I could make reference to it while editing, but the list is there which is great.

I do wish that there was a bit more functionality for the output of a message where one could specify a format without having to specify each and every X for {field_name_X} and {field_value_X}. Having the values able to be specifically pulled is fantastic, but having a general format one could have that would apply to every question within that form would be much more utilitarian.

I also notice that when using a selection box to eliminate some questions (e.g. a staff application wherein you select a specific team and it populates the questions based on that), the {field_list} will still output the 'hidden' questions, which doesn't look good in a final format.

Lastly I wish there was a way to enforce a certain tag depending on a selection box much like how field hiding works with that.

All in all if you need a form, this is a great solution, though for very specific niche uses of the features presented, it can be a bit clunky.

We use Forms as a way to have new members introduce themselves on our site. They fill in a simple form explaining how they found us and why, and telling a bit about ourselves. That gets posted to a "new members intro" forum in a nice format, and triggers an email and a few other things. We drive new users to the correct Form through a custom plug-in we've written that takes control after initial user sign-up.


We're really happy with Forms. It's well worth the yearly renewal fee.

This application works pretty well and was a breeze to setup, but there seem to be some issues:

  • Attachments don't always upload properly, links to non-existent attachments can appear in field lists. This is a major issue.
  • Custom URL slugs don't always work properly, and can reset to their default when editing forms.

Overall I think it is a good application, it is pretty easy to setup a form to collect information from members and make the site more interactive.  It has all the major form fields you need to get the job done.

Here are a few things that I think can be improved:

  • Would be cool to have an easy Preview Link to see how your form looks
  • Would like to easily be able to direct link to a form.  Would be cool to just give you the link in the admin area of where the form can be located
  • Have the ability to not have a Forms page that people can look at all the forms you created.  I guess there are people that can use this, but I just need to create a form for particular programs and don't need to show a list of forms so it would be great to remove that button in the settings and not link to ALL forms
  • Plugin is $40 with $20 maintenance.  I am fine with that, but don't include a copyright watermark.  It looks cheesy on my site and I don't care to include this and want to pay more to have it removed.

Rules support! Sweet! Now you can do nearly anything with this app. I'm totally stoked. Now I can build automated member group change request and fulfillment forms, topic close request forms, and attach all sorts of new behaviors to custom forms! Plus this app logs every submission nicely. Love that.

This is a nice add on to my forum.

However, im wondering if there is a way to use the field values defined in the form, can be used within the forum topic or the pm? If that would already work, i didn't find a way. If not, it would be a great addition, personally im using the forms for my members to create ban appeals or requests. Having a directly created forum topic with the specified values will totally simplify the way for them.

Edit: I just found the way how to get field values. It would be good, adding at least a small documentation. Also the quick-form-field-names like in the previous IPS version is not displayed, which would simplify it a bit.

This is one of the most important parts of my community. With well over 60 forms that we have on our site, it makes everything clean and easy for submissions. Stop using other forms, Google, Office365, etc . and use this for your forums!  

It's well worth the $40 plus $20 per year for what you get from this app and the support that Mike gives! Great guy and great developer.

Absolutely a must have for anyone looking to rid themselves of things like google forms and want to integrate forms completely in to their website.

I love this app and have made great use out of it. Easily my best purchase for my IPboards website.

The forms does not work with upload function, pretty serious bug. It seems to only work with images, zip/rar/documents do not work. I would like to have a CHKEditor option as well.

A tad on the expensive side but honestly worth it. I used this to make a whitelist application for my gaming server, ban appeals, and even am working on staff applications. This essentially lets you create beautifully formatted forms from a template without having people just copy / paste an app into a thread. Really useful. 

Also, support was really fast. I was a little confused and Mike John explained exactly what to do within a few days. Plus he even coded in a feature I requested!

First of all this app is amazing, works perfectly for the usage i need it for. Sadly i had a issue where on a post it wont show the Time, but i got instant support within 12 hrs and could fix the issue to be able to add the Time post function to the Forms posted. Awesome

I hope with a future update the time will be able to appear next to the date. A short quick fix would be as suggested from the Devloper posted on the Screenshots added.

Plugin works great and does what it should except these things:

  • Form list throws an error and is unusable, even on the default unmodified theme.
  • When you choose to create a topic on form submission, image links do not embed.
  • Form submissions override forum permissions. (for example: post approvals will be ignored and content will be posted without approval)

Those things make the plugin somewhat useless for me or require me to work around it which is a shame for that price.

Just playing with it now and I think it works great. 

Not sure if this is a difficult thing to implement or not.... but a spacer field?  Like a horizontal rule and/or Caption/Title? Something to help group things?

Would it be possible to use field values for tags and prefixes when allowing form tags or prefixes, or when creating a topic using a form?
